Form ID

If you already have your Form ID enter the same to pull the saved data, otherwise enter your preferred Form ID which can be used later to pull all your saved data.

Upload the site survey or assessment detailing the following characteristics of the site:

Topography
Climate
Soils
Human health effects

Hydrology
Vegetation
Human use

Upload the completed Site Assessment Worksheet (found under the credit's "Resources" tab in the Credit Library) or an equivalent narrative description.

Upload a map illustrating the topography of the site.

Upload a map illustrating the site's Special Flood Hazard Areas (SPFHA) as determined by FEMA's Flood Insurance Rate Map (FIRM) (or local equivalent showing the 100-year floodplain for projects outside the U.S.).

Provide the description of the site's EPA Level III ecoregion (or local equivalent).

Special Circumstances

Describe the circumstances limiting the project team's ability to provide the submittals required in this form. Be sure to reference what additional documentation has been provided, if any. Non-standard documentation will be considered upon its merits. (Optional)
